Team Leader - Supported Living

Job Description

21 hours during weekdays/ends

To support the Service Manager and lead the care team during any span of duty, ensuring a safe effective and efficient service is maintained which meets the needs of the clients and commissioners, within budgetary constraints, all relevant legislation, the seven principles of conduct and the policies and procedures of the Trust.

  • To maintain skills and knowledge and ensure that care is delivered according to the latest guidelines and best practice evidence available.
  • Liaise with GPs and other health professionals to ensure that clients receive the medical and clinical support they require.
  • Ensure effective and clearly documented handover systems are maintained to facilitate the effective transfer of information between staff and shifts.
  • Attend and participate in Team and staff meetings as arranged by the Registered Manager or Service Manager.

  • Understand the requirements of the Mental Capacity Act (2005) and Deprivation of Liberty Safeguards (2008) in relation to consent and best interest decision making.

  • Provide information to clients and relatives in relation to care delivery, being mindful of data protection and Caldicott requirements.

  • Ensure high standard of record keeping are maintained in accordance with the Health and Social Care Act (2008) and Essential Standards of Quality and Safety (2010) and Company requirements

  • To ensure all clients receive support to complete and implement their individual support plans, supervise and monitor progress of specific support goal delivery and support including assistance with shopping, accessing community facilities, medical appointments and other activities as directed by the Service Manager.

The post holder will be required to present a good image of the Trust as well as maintaining constructive positive Internal relationships with all Trust employees, The Board of Management and tenants, customers, their relatives and carers; Local Authority Commissioning staff; Health and Social Care professionals; Regulatory representatives; Independent Providers; Voluntary organisations; Emergency Services and all agencies, organisations or customers at all times.
